Rachel Smith is a Mental Health Counselor and works as a Behavioral Health Consultant (BHC) on our Integrated Behavioral Health team. BHCs at Community Care Physicians are licensed mental health providers and social workers. Many are embedded within our practices working side by side with your doctor and health care team in delivering you the best “whole person” healthcare possible.
Rachel is a compassionate and dedicated mental health professional who focuses on helping individuals navigate their behavioral health journeys. She has gained extensive experience working with diverse populations in community settings, where she provides individual, couples, and group counseling.
Rachel has experience treating a multitude of behavioral health issues including, but not limited to, depression, anxiety, PTSD, grief, substance abuse, and behavioral intervention for weight management helping patients create sustainable plans to reach their goals, addressing potential roadblocks, developing strategies to overcome challenges, and maintaining motivation. Rachel utilizes different interventions including Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), taking a person-centered and strengths-based approach. She also co-facilitates a weekly group, focusing on goal setting and treatment planning to empower her clients.
Rachel’s interest in psychology and human behavior began in high school when she first explored the subject. Her innate empathy, strong listening skills, and desire to help others led her to pursue a career in counseling. She finds great satisfaction in building trusting relationships with her patients and supporting them as they work towards positive change.
In her free time, Rachel enjoys outdoor activities like hiking, running, and skiing and values spending time with her family.
